Services and Costs NOT Included (Exclusions)
MEMBER ACKNOWLEDGES THAT THE MEMBERSHIP PACKAGE IS TELEMEDICINE-ONLY:
- No In-Person Care: No physical exams, vaccines, or stitches.
- Third-Party Costs: Member is responsible for all external costs for labs, imaging, and
Term, Cancellation, and Re-Enrollment
Unlimited virtual consultations as medically necessary.
- Cancellation: 30-day written notice required.
- Early Cancellation Fee: If cancelled within the first 90 days, a fee equal to one month’s membership will apply.
- Re-Enrollment Policy: Patients who cancel and wish to re-enroll within 12 months must pay a Re-Enrollment Fee .
